About the Role
Employee strives to meet the needs of all customers while supporting the VA mission. Consistently communicates and treats customers (Veterans, patients, families, Veteran representatives, visitors, volunteers, and all VA staff) in a courteous, tactful, and respectful manner. Provides the customers with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the customer constructively and appropriately.
What You'll Do
- →Health and Wellness Coaches provide services to Veterans seeking self-directed, lasting changes aligned with their values.
- →They provide support that promotes health and enhances well-being, which may support improved health related outcomes, and quality of life.
- →Health and Wellness Coaches work with individuals on identifying their motivation for change, determining goals for their work together, developing sustainable action steps toward achieving their short- and long-term goals, implementing action plans, and assessing progress in order to adjust plans as necessary.
- →Health and Wellness Coaches provide support to positively influence the relationship between lifestyle and health outcomes.
- →They guide the exploration of behaviors that may increase the risk of developing or worsening chronic disease and those that reduce the likelihood of chronic illness.
- →Assumes responsibility in work assignments in accordance with regulations and independently resolves most conflicts by using considerable judgment and interpretation .- Work is reviewed to ensure that overall objectives of the position are met.
- →Health and Wellness Coaches at the entry level (GS-7) will require closer supervision.
- →As they are providing coaching, they may seek supervisory support or mentorship to address complicated questions or issues that may arise.
- →Provides care and/or services appropriate to the age of the patients being served, and which is appropriate to the cognitive; emotional, and chronological maturation of the patient.
- →Assesses data reflective of the patient's status and interpret the information needed to identify each patient's requirements relative to their age-specific needs and to provide care needed as described in the services' policies and procedures.
- →Takes into consideration age-related differences of the various veteran populations with this institution.
- →The incumbent must demonstrate the knowledge of the changes associated with aging and possess the ability to provide care/support based upon age related factors (as below).
- →Sensitivity to the special needs of all patients in respect to age, developmental requirements, and culturally related factors must be consistently achieved.

Requirements
- ✓You must be a U.S.

- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Must be proficient in written and spoken English.
- ✓Subject to background/security investigation.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
