About the Role
General Description of Assigned Duties: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.
What You'll Do
- →Functions or Scope of Assigned Duties: The Nurse I, Level 1 Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal outpatient care.
- →This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program.
- →Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
- →This nurse plans and delivers technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
- →Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care.
- →Makes home visits to Veterans in the program.
- →The focus is on individual growth and development in HBPC nursing practice with increased levels of self-direction.
- →Seeks guidance on how to improve care provided to patients and families.
- →Delegates elements of care to the appropriate individual based upon assessed needs, condition, and safety of the patient.
- →The nurse brings HBPC principles and theory with limited application of critical thinking skills.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.
