What You'll Do
- →Serves as the installation non-clinical Case Worker for the USMC Exceptional Family Member Program (EFMP).
- →Manages services including intake, identification, assessment, evaluation, tracking, and referral of eligible family members.
- →Acts as a liaison with medical, non-medical, installation, and community professionals to ensure top-tier care aligned with national and Marine Corps standards.
- →Core Responsibilities : Case Management: Conducts assessments, completes Family Needs Assessments and Services Plans, and establishes/maintains electronic case files with detailed contact notes in the EFMP Case Management System (CMS).
- →Family Support: Contacts each EFMP family upon assignment notification.
- →Routinely assesses circumstances, assists families in creating and achieving goals, and empowers them to pursue available resources.
- →Resource Navigation: Connects families to vital services including Respite Care, Special Education, Supplemental Security Income/Social Security Disability Insurance, Medicaid Waivers, Early Intervention, and Tricare.
- →Advocacy & Collaboration: Participates in Individualized Education Plan (IEP) and Inclusion Action Team (IAT) meetings per MCO 1754.4.
- →Contributes to a library of special needs educational and referral materials.
- →PCS & Transition Assistance: Provides "warm hand-off" assistance during Permanent Change of Station (PCS) relocations.
- →Coordinates with losing/gaining case workers and advises families on relocation options (e.g., assignment reconsideration, Continuation of Location).
- →Compliance, Safety, & Administrative Duties : Information Security: Safeguards highly sensitive, proprietary, and privileged data.
- →Ensures strict compliance with the Privacy Act (5 USC 552a), HIPAA (PL 104-191), and DoD Directive 6025.18.
- →Restricts unauthorized access or dissemination and reports data releases immediately.
