About the Role
Demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines.
What You'll Do
- →Responsible for all elements of the nursing process providing competent, evidence-based care to residents with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting.
- →The LTC RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct Veteran care.
- →The LTC RN is expected to communicate and interact appropriately and courteously with all internal and external customers.
- →Assesses, plans, implements, and evaluates care based pm age specific components.
- →Administers medications and procedures per established policies and guidelines, influences care outcomes by collaborating with members of the interdisciplinary team.
- →Facilitates communication among licensed and unlicensed staff, the physician, the Veteran, their family, and other department personnel, as needed.
- →Utilize the nursing process and evidence-based practice in order to develop individual plans of care for patients in the CLC.
- →Community and Living Center (CLC) Staff RN considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences.
- →Administers medications and procedures per established policies and guidelines, influences care outcomes by collaborating with members of the interdisciplinary team.
- →Assumes responsibility for the coordination of care focused on patient transition through the continuum of care.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
