About the Role
The Community Living Center (CLC) Nurse Manager (NM) functions as a clinical expert and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care. The NM assesses, plans, implements, and evaluates delivery of care based on age specific components, and assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction by overseeing the CLC program.
What You'll Do
- →The CLC NM provides leadership and supervision for the nursing staff of the long-term care and hospice units within the SFVA CLC.
- →The NM ensures delivery of compassionate, evidence-based care that supports comfort, dignity, symptom management, and psychosocial-spiritual needs for Veterans and their families.
- →The position includes administrative, clinical, and operational responsibilities such as staffing, budgeting, performance improvement, regulatory compliance, and program development.
- →The NM collaborates with physicians, advanced practice providers, and leadership across Geriatrics and Extended Care programs to promote seamless transitions of care and continuity across inpatient, outpatient, and community setting Duties and responsibilities include: Oversees daily administrative and clinical operations of the assigned unit.
- →Formulates and implements policies and procedures.
- →Keeps updated on CMS Long-Term Care trends, developments, and regulations.
- →Ensures continuous accreditation readiness.
- →Monitors quality metrics and drives performance improvement activities, including SAIL and CLC.
- →Participates and serves as Chair/Co-Chair in committees and task forces to enhance nursing practice and patient care.
- →Ensures customer service, patient satisfaction, employee safety, and a healthy work environment.
- →Maintains 24/7 staffing levels and meets nursing hours per patient day requirements.
- →Promotes education, professional growth, mentorship, and clear role expectations.
- →Supports student nurse clinical experiences and orients new Nurse Managers.
- →Assigns a Nurse Manager for administrative coverage in their absence.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
