About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

- →Independently manages Veterans under discretionary contracts awaiting SVH placement.
- →Coordinates with SVH admission staff to ensure timely and appropriate placement.
- →Reviews Veteran Directed Care consults, performs biopsychosocial assessments, determines eligibility, and completes required documentation.
- →Provides expert guidance to Veterans, caregivers, and interdisciplinary teams.
- →Uses the Case Mix Tool to determine service hours and collaborate with stakeholders.
- →Provides specialized case management for STR Veterans needing enhanced services, particularly those at risk of homelessness.
- →Submits and monitors referrals to GEC administrative staff for financial authorization processing.
- →Visits Veteran patients at CHNs to conduct thorough psychosocial assessments and make psychosocial diagnoses.
- →Provides high-quality case management and care coordination to Veterans residing at CNH or instructional settings.
- →Independently works with Veterans and their families who are experiencing a variety of psychiatric, medical, and social utilizing individual/family counseling.
- →Assists CNH staff with determining appropriate care planning including discharge planning as needed.
- →Assesses CNH Veterans' functional status and fitness for continued placement.
- →Address all CNH resident/family concerns identified.
Requirements

- ✓Must be proficient in written and spoken English.
- ✓Subject to background/security investigation.
