About the Role

The Cincinnati VA Medical Center is looking for an Outpatient Clinical Pharmacy Specialist (CPS), licensed professional with a scope of practice who works collaboratively serving as a medication expert to assist primary care and specialty care teams in meeting the medication therapy needs of their patients. The CPS functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services.

What You'll Do

  • The Cincinnati VA Medical Center (CVAMC) is a two-division campus located in Cincinnati, OH and Fort Thomas, KY serving 15 counties in Southwest OH, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH.
  • The CVAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division.
  • Duties Clinical: MH CPS evaluates, prescribes, modifies, or discontinues medication therapy and acts as a consultant for mental health pharmacotherapy working under a scope of practice and care coordination agreement.
  • Follows national VA guidance for screening and completion of the VA Comprehensive Suicide Risk Evaluations.
  • When significant risks are identified, the MH CPS will assist in suicide prevention planning and/or refer to appropriate clinicians when needed.
  • Completes clinical reminders and screenings.
  • Develops, implements, and maintains Clinical Pharmacy Clinics as assigned.
  • Performs population management utilizing dashboards and metrics to identify patients who could benefit from MH pharmacist services.
  • Serves as a subject matter expert resource in mental health pharmacotherapy for the Medical Center.
  • Additionally, acts as a liaison between Pharmacy Service and Mental Health Care Line to assist with patients' mental health related management needs.
  • Works with providers to ensure compliance with national, VISN, and local initiatives, and promotes and monitors compliance for established drug therapy policies.
  • Serves as an authority on drug usage, interactions, over-dosages, and medication compliance to facility staff.
  • Uses appropriate references to research drug information for providing timely and accurate responses to drug information questions.
  • Participates in the focused and ongoing professional practice (FPPE & OPPE) evaluation process.

  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
