About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

What You'll Do

  • The Clinical Pharmacist will be responsible for providing a variety of pharmaceutical services.
  • These services include but are not limited to reviewing, interpreting, and verifying medication/supply orders for both Inpatients and Outpatients.
  • Processing and filling medication orders in an accurate and timely manner.
  • Dispensing, compounding, and preparing a range of medications including unit dose, sterile products, ophthalmic, narcotics, investigational drugs, oncolytic agents, etc., for eligible Veterans.
  • Duties include, however, not limited to: Recommends appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics.
  • Monitors for efficacy, side effects and clinical outcome.
  • Provides patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendation is to other health care providers in charge of the patient both orally and in writing.
  • Monitors include prospective review and intervention.
  • Performs continuous evaluation of prescribed medications to assure optimal drug therapy.
  • Provides medication counseling to patients.
  • Serves as a drug information resource by providing up-to-date drug information to prescribers, other health care professionals, patients and caregivers.
  • Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS reporting program.
  • Recommends formulary alternatives as appropriate.
  • Provides pharmacokinetic consultation, including dosing vancomycin and aminoglycosides, in accordance with local policy.

  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
