About the Role
The Registered Nurse (Outpatient Mental Health Access RN) carries out responsibilities that demonstrate leadership, experience, and creative approaches to managing complex primary care. This nurse provides gender-specific direct Veteran care and care coordination, as appropriate, for Veterans receiving care within the Primary Care Department.
What You'll Do
- →A Career At VA Salt Lake City Health Care System Can Take You Places (youtube.com) Duties and responsibilities include, but are not limited to the following: Facilitates access into Outpatient Mental Health (OPMH) ensuring timely access to care which may include intakes for medication management and psychotherapy, transfers of care, and triaging needs for other services.
- →Conducts consult management including triage and disposition, utilizing CPRS, Consult Tracking Management, and other applicable computer applications and/or dashboards.
- →Coordinates appointments which may include scheduling.
- →Collaborates frequently with Referral Coordination Team (RCT), Care in the Community (CITC) and Health Administrative Services (HAS) to streamline assessment for drive-time and wait time eligibility, community referrals, and VA scheduling.
- →Collaborates with various OPMH programs/teams, including CBOCS, and the VA facility at large to facilitate access to clinically appropriate OPMH services.
- →Liaises with Inpatient Psychiatric Unit and Psychiatry Consult and Liaison Service to ensure seamless transition from inpatient to outpatient care.
- →Provides regular consult status reports to MH Executive Leadership via supervisor.
- →Participates in relevant meetings and workgroups as it relates to consult management.
- →Facilitates outpatient psychosocial/psychoeducational groups for Veterans.
- →Provides coverage for OPMH RN case managers and other RN roles in OPMH as needed which may include medication administration, care coordination.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
