About the Role
The Home and Community Based Care Outpatient Staff Registered Nurse provides competent, evidence-based nursing care to Veterans across outpatient and home settings. The RN applies the nursing process, performs assessment, triage, case management, patient and caregiver education, medication and treatment coordination, and care planning; supports safe transitions, self-management, customer satisfaction, interdisciplinary collaboration, quality improvement, and evidence-based practice.
What You'll Do
- →This position is part of the bargaining unit.
- →Nurse I, Level 1 Provides fundamental, knowledge-based outpatient nursing care to assigned Veterans.
- →Assesses, plans, implements, and evaluates care using the nursing process with close supervision.
- →Provides patient education, self-management support, and transition planning.
- →Administers medications and treatments according to policy and procedure.
- →Formulates plans of care based on nursing assessments and patient needs.
- →Delegates care tasks appropriately based on patient condition and safety.
- →Participates in evidence-based practice, education, and performance improvement activities.
- →Nurse I, Level 2 Provides competent, evidence-based H&CBC outpatient nursing care to Veterans.
- →Performs case management, assessment, triage, education, resource coordination, and care planning.
- →Coordinates medications, treatments, and services for Veterans with complex care needs.
- →Provides care in the home setting, including health promotion, prevention, geriatric, primary, and palliative care.
- →Conducts home visits for Veterans enrolled in H&CBC programs.
- →Supports safe care transitions with Veterans, families, and the interdisciplinary team.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.

- ✓The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).
