About the Role
The VA Texas Valley Coast Bend Health Care System, Harlingen, TX is seeking a Registered Nurse Case -Caregiver Support Coordinator (Geriatric Service) to assists family caregivers with the psychosocial and emotional stressors of caring for an ill, injured, or disabled Veteran, as well as provides resources linking the caregiver to VA health care and community services.
What You'll Do
- →The Registered Nurse (Caregiver Support Coordinator-Geriatric Services) will completes a comprehensive clinical assessment at the initial home visit/evaluation, coordinates referrals to VA and community-based resources and programs and provides ongoing evaluation of services every 120 days.
- →The Caregiver Support Program (CSP) RN also completes Veteran Functional Assessments for Veterans and Home Care Assessments for Veteran and listed Caregivers for in process applications via VVC.
- →Functional Assessments can be completed in clinic; Home Care Assessments must be completed in the home.
- →Evaluates patients across all six counties covered by the VATVCBHCS.
- →Assesses patient's and caregiver's progress, caregiver burden, and home care needs at each visit.
- →(Wellness visits are scheduled every 120 day, 1 visit needs to be face-to-face, the other 2 visits can be completed via Telephone or VVC).
- →Visits are documented on National CG templates for both Veteran and all Caregiver(s) listed.
- →Assesses, coaches, and supports the caregiver to perform the skills necessary to deal with the Veteran's mental illness needs.
- →Provides education and in-service training to VA Health System employees, Veterans, family, and Caregivers in the CSP.
- →Performs a periodic home visit to Caregivers and Veterans who are not receiving a full home care program from the VA but require a review of the skills and abilities of the Caregiver.
- →Works directly with the VATVCBHCS CSP Manager for VATVCBHCS to facilitate caregiver support groups and educational programs.
- →Demonstrates advanced clinical knowledge in assessing planning, implementing, documenting, and evaluating care for a designated group of patients.
- →Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems.
- →Collaborates with the interdisciplinary team, patients, and care providers.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
