About the Role

This position is in the Clinical Resource Hub (CRH) at the VA Connecticut Healthcare Systems, located in West Haven, CT. This position is full-time at 40 hours per week.

What You'll Do

  • The Clinical Resource Hub (CRH) Telecardiology Registered Nurse (RN) provides advanced clinical, technical, and programmatic support essential to delivering high-quality virtual cardiac care across the Veterans Integrated Services Network (VISN).
  • The Telecardiology RN demonstrates leadership, independent clinical judgment, and innovative approaches to managing complex cardiovascular conditions in a virtual environment.
  • Duties include, but are not limited to: Supports all VISN 1 Hub locations participating in the delivery of virtual cardiology services, including VA medical centers, community-based outpatient clinics, and provider alternate worksites, ensuring seamless coordination across distributed care environments.
  • Manages incoming VISN 1 CRH cardiology consults (general cardiology, heart failure, arrhythmia, Zio patch, Echo consults and E-consults), ensuring timely review, accurate triage, appropriate assignment, and coordinated follow-up consistent with national and VISN consult management standards.
  • Provides enterprise-wide support to the VISN 1 CRH by delivering synchronous and asynchronous telecardiology services to Veterans within and outside the VISN, as needed.
  • Provides direct virtual patient care to one or more panels of cardiology patients, including those with coronary artery disease, hypertension, heart failure, arrhythmias, dyslipidemia, and structural heart disease.
  • Conducts remote cardiac assessments using telehealth technologies; interprets home-monitoring data (blood pressure, weight, heart rate, symptom trends), reviews diagnostic data (ECG, Zio reports, echocardiogram results), identifies early signs of clinical deterioration, and escalates concerns based on clinical pathways and protocols.
  • Coordinates cardiac diagnostic workflows, including echocardiography scheduling, Zio patch placement/logistics, stress test coordination, follow-up visit triage, and all telehealth visit workflows for cardiology patients receiving virtual care.
  • Engages Veterans and caregivers in shared decision-making, offering education on cardiovascular disease management, medication adherence, lifestyle modification, symptom monitoring, home BP and weight tracking, and heart-failure self-management strategies.
  • Delivers telephone- and video-based clinical triage, responding to patient concerns, assessing symptoms, identifying urgent cardiac red flags (e.g., chest pain, rapid weight gain, arrhythmias), and ensuring timely connection to appropriate cardiology or acute-care services.
  • Provides comprehensive care management and coordination, monitoring high-risk patients particularly those recently discharged after heart-failure or cardiac-related hospitalizations to ensure timely follow-up, early intervention, and reduced readmission risk.
  • Supports and collaborates with CRH and spoke-site staff on telecardiology workflows, virtual-care technologies, remote monitoring platforms, and best practices in virtual cardiovascular nursing.
  • Exempted/Mid-term extension positions are reviewed regularly and do not imply permanent remote status.
  • Telework: This is not a telework position.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
